New Patients

All new patients joining the Practice will be asked at point of registration to provide two proofs of identity, one should be photographic and at least one should indicate your current address.

You will be asked to attend the surgery fir a brief review of past medical history.

A form outlining you past medical history will be provided and the Practice Nurse will assist with any problems in completion. This allows the doctors to assess your medical needs and record any medication prescribed by your previous doctor. It also allows the patient to ask any questions regarding the practice and become familiar with the staff here.

This service is available to all new patients, except children under 5 years, who will be asked to attend the child health clinic. Each patient is registered with a named doctor, but all our patients have the choice of seeing whom they wish within the Practice.
