Health Care Assistant Clinic

Diabetic Clinic
Diabetic patients will be invited depending on their individual diabetic requirements. We request blood tests are taken in the week prior to these appointments in order that the nurses have up-to-date results at hand, and would also ask that patients pick up a white top urine sample bottle to be brought with them at the time of their Clinic appointment.
One of our District Nurses also provides this service to housebound patients.
Long-Term Conditions Clinic
This clinic is aimed at those patients with Long Term Conditions such as Heart, Kidney or Respiratory illness. Patients are requested to make an appointment for blood tests in the week prior to their clinic appointment to ensure their results are available for review. You will be informed when making your appointment.
