Patient Registration

You will be asked to attend the surgery for a brief review of past medical history.

All new patients are required to complete a New Patient Registration Form.  All new patients joining the Practice will be asked at the point of registration to provide proof to indicate your current address. The Practice has a set boundary where patients will/can be accepted from.  Please check with our Administration Team if your address meets our boundary criteria agreed with NHS Ayrshire & Arran.

A form outlining you past medical history will be provided and the Practice Nurse will assist with any problems in completion. This allows the doctors to assess your medical needs and record any medication prescribed by your previous doctor. It also allows the patient to ask any questions regarding the practice and become familiar with the staff here.

Each patient is registered with a named doctor, but all our patients have the choice of seeing whom they wish within the Practice.

Change of Personal Details

Please ensure that you inform us as soon as possible of any change of address/telephone number.

This is particularly important if you are awaiting hospital treatment as occasionally urgent contact may be required.

Registration Form

Please print off both forms and bring to the practice with Identification please.  You will them be added to our system and given a new patient check with our Health Care Assistant.
